You must call our office at 336-236-1675 to initiate an intake over the phone.
Step 2.
After you have called us to complete an over the phone intake, please click the link below and gather all required information so that we can have everything prepared for an office visit. NOTE: We cannot see you unless this packet has been completed and all required attachments are included.
Step 3.
After you have gathered all of the information needed in Step 2,
please call our office again at 336-236-1675 to confirm your packet is complete.Note: If you tell us your packet is complete, and we schedule you an appointment, you will be rescheduled if anything is missing.
